First - schedule an appointment with me by calling 651-294-2340 or clicking on Appointment Request off the main menu. Once you have an appointment scheduled, please register by clicking the link below. This will get all your demographic/insurance information input into my system before our appointment and allows us to utilize the appointment time fully on your needs.
Please note that I use a billing service that is embedded in another clinic. You can simply select my name from the drop down list of providers and follow the prompts to complete your information.
